Abstract

Software testing documentation practices are essential in regulated projects where software quality, safety, compliance, and audit readiness must be demonstrated through clear evidence. In domains such as healthcare, banking, aviation, insurance, and government systems, testing cannot depend only on informal validation or developer confirmation. It requires structured test plans, test cases, traceability matrices, defect logs, test execution records, approval notes, and final test summary reports. This article discusses how testing documentation supports verification, validation, compliance control, and accountability across the software lifecycle. It explains the role of requirement-to-test traceability, version-controlled test artifacts, defect evidence, review records, and acceptance criteria in maintaining reliable quality assurance. The article also highlights common challenges such as incomplete test records, outdated documents, weak audit trails, inconsistent defect classification, and poor linkage between requirements and test results. A structured documentation approach is presented to improve transparency, reduce compliance risk, support audit inspections, and strengthen release confidence. The study concludes that disciplined testing documentation improves project control, supports regulatory evidence, and ensures dependable software delivery in regulated environments.
